Program Provisions and Exclusions

Our plan is designed to make dental care affordable, but it is not insurance. Here are a few important things to know before you enroll.

  • The annual enrollment fee is non-refundable and due at your first appointment, with no exceptions.
  • Two exams and cleanings must be used within the enrollment year; visits do not roll over.
  • Patients are responsible for scheduling their own appointments throughout the year.
  • A $50 fee applies to broken appointments without 24 hours of notice.
  • The plan is non-transferable to family members and cannot be combined with insurance or other offers.
  • This plan is not insurance and cannot be used with workers' compensation claims.
  • Care is limited to services provided in our office; referred specialist visits are not covered.
  • Orthodontic treatment requires enrollment for the full course of care; annual rates may change.
  • Payment for additional treatment is due at the time of service.
